2016-2017 Immunization Requirements

On Tuesday, May 31st Beverly Katovich from Panhandle Health will be at KHS for a Shot Clinic at 8:30 a.m. Forms can be picked up at the office giving permission to be immunized. Idaho law requires all incoming 7th grade students to have proof of and adolescent whooping cough booster shot, also known as Tdap, a meningococcal shot, or file an exemption in order to attend school in the fall. Remember, in addition to the 7th grade shots, there are several important vaccines recommended for preteens and teens including varicella (if you have never had chickenpox disease), hepatitis A, the HPV vaccine series, and a seasonal flu shot.

Vaccines given will be billed to Insurance or Medicaid. If neither of these apply the cost of vaccine will be billed to parents at $20 per vaccination up to a maximum of $60 no matter how many vaccinations are given.
